How to Verify Insurance for Suboxone Treatment

Insurance verification starts with the exact product on your current card. Aetna commercial coverage, for example, is different from Aetna Better Health Medicaid. A provider’s participation can also differ by state.
Before you book
Have your name as shown on the policy, member ID, plan name, and current coverage information ready. Grata asks for ID and insurance information before the first appointment. Our team helps check benefits and explain the expected visit cost.
What Grata accepts
Grata in Virginia
Grata accepts Aetna Better Health of Virginia, Anthem HealthKeepers Plus, UnitedHealthcare Community Plan, Humana Healthy Horizons of Virginia, Straight Virginia Medicaid (fee-for-service). Care is $0 with accepted Medicaid coverage.
Medication and individual therapy are available. Therapy is optional for medication treatment.
Grata in Ohio
Grata accepts Anthem Blue Cross and Blue Shield Medicaid, Humana Healthy Horizons in Ohio, Straight Ohio Medicaid (fee-for-service). Care is $0 with accepted Medicaid coverage. UnitedHealthcare / Optum participation is commercial only; do not assume Medicaid coverage.
Medication and individual therapy are available. Therapy is optional for medication treatment.
Grata in Pennsylvania
Grata accepts Straight Pennsylvania Medicaid (fee-for-service Medical Assistance). Care is $0 with accepted Medicaid coverage. Medicaid managed-care plans are not currently accepted.
Medication care is available. Therapy is not yet available in Pennsylvania.
Most appointments are available the same day with the clinician who will treat you. Medication can be prescribed at that visit when clinically appropriate.
Commercial plans are listed separately on our insurance and pricing page. If a product is not listed, ask the team to check it before relying on coverage.
Check the clinician visit
Ask which practice and clinician will provide care, whether they participate in your exact product, and what you will pay for the visit. Confirm the service you want: medication management and individual therapy have different availability and billing. Pennsylvania currently offers medication care without therapy.
Care is $0 with accepted Medicaid. Commercial copays depend on your plan and are usually less than $20 per session. Self-pay prescribing care is $199/month, excluding medication and therapy.
Check the pharmacy separately
Your prescription benefits apply at the pharmacy. Ask about the prescribed medication, formulation, quantity, pharmacy network, and any authorization requirements. Grata can help resolve access problems. Delivery takes 1–2 business days from prescribing, with covered shipping; local pickup is also available.
When coverage changes
Tell the care team before your next appointment if your plan changes or coverage ends. Keep written information from the plan, including a denial notice and its instructions if applicable. Do not assume commercial participation means participation in a Medicaid product with the same brand name.
